Volunteer

Each parent or adult family member who will be volunteering in the classroom and/or driving on field trips is required to fill out a volunteer application form. Each year, new and returning parents must review and update indicating any change on the the volunteer application prior to the first work day in the classroom. As part of the District policy to ensure safety of all students, background checks for volunteers are mandatory. 

A fingerprinting process will also be mandatory of any parent or adult volunteer who will be participating on any aspect of an overnight field trip including any portion of the day or night of the field trip. This process is administered at the District Office on Lawrence Road. All results are confidential, and the District office does not share any details of the background checks with any staff or volunteer parents at our school. 

Volunteers must comply with the directions of the School Administration district COVID-19 requirements. 

If you are a new volunteer this year, please note that filling out the form below will trigger District Office personnel to reach out to you to schedule an appointment to be fingerprinted at the District Office (1889 Lawrence Road, Santa Clara). Please make your appointment as soon as you receive the email so that you can be cleared and begin volunteering in the classrooms.

Here are some steps to guide you through the volunteer process:

1. Complete the online volunteer packet below, selecting the school or department from the drop-down menu (fields in red are mandatory):

2. Submit a copy of a current TB test.
This form will need to be completed by your doctor, and emailed to the Secretary or turned in at the office.

The school will review the application and either approve or deny the request. The school will notify you when you are cleared to volunteer.

Once you are cleared to volunteer, the Office will take your photo at school and create a ID badge for you to wear when you are on campus at all times.

First and foremost, you will need your volunteer packet filled out and processed by the District, as well as have your fingerprinting completed.

1. In the beginning of the year, you will need to turn in your Driver's Record. To get a copy of your Driver's Record, you will need to request it on the DMV's website and pay a $2.00 fee online. DMV Driver record must be dated within 30 days, and submitted annually for all volunteer drivers.

Each driver must have the required insurance forms and policies on file in the office. The District mandated car insurance is a minimum of $300,000 liability, $100,000 bodily injury, and $50,000 property coverage. Drivers must also send copies when the policies are renewed/previous policy expires. Your class Insurance Coordinator will remind you. 

Note: If a field trip occurs on your classroom work day, we plan on work day parents driving on the field trip. If this is not possible, please consult with your teacher or Field Trip Coordinator for possible options, such as: 

• Participate as a passenger "rider"/chaperone. 
• Trade work days with another parent who can drive on the field trip.
