Washington Open 2024-2025 Volunteer Application 

If you are a new volunteer this year, please note that filling out the form below will trigger District Office personnel to reach out to schedule an appointment to be fingerprinted at the District Office (1889 Lawrence Road, Santa Clara). Please make your appointment as soon as you receive the email so that you can be cleared and begin volunteering in the classrooms.

Share this form with your doctor to fill out, and return to the office.

2024-2025 Driver's Insurance Cover Form (PDF)

Print, fill in, sign and submit with required attachments.
 

California DMV Driver Record Request 

Complete online DMV form, and return printed results to the office.

Planned Absence Notification

If you know your child will be absent (e.g., for religious observances, family vacation, etc.), please fill out this form and submit it to the Office.

At School Medication Procedure

Should your child need medication during school hours a medication permit form needs to be completed and signed by your child's doctor.

The completed form and medication is to be brought to school in its original container. Medication will be stored and dispensed from the health office.

 

P.E. Class - When Your Child is Injured

PE Modification Form

If your student experiences an injury that temporarily limits their physical ability, they may continue to participate in Physical Education class through modified activity.

Please have the doctor provide a note indicating what specific activities your child can do, or have the doctor complete and sign the form linked below. Your child should then give the form to the school office.
